Payment is
due at the time of service, unless we are billing insurance or other
arrangements have been made. We accept cash, check or credit/debit cards. Always give cash directly to the office staff and get a receipt. Insurance

We provide third-party billing to insurance as a courtesy to our patients. After
verification of benefits, you will be told of a co-pay amount, if any. Please pay co-payment at the time of visit. Auto Insurance – Payment is generally not required since insurance pays at 100 percent in most cases. You are, however, responsible for deductibles or other uncovered items. Nutritional
supplements and any supplies must be paid
for in
full at time of purchase. You will be reimbursed if insurance covers them. Appointments
We ask that you notify us as soon as possible when you know you have to modify your appointment time in any way. If your call goes to voice mail, please leave your message with reception and not on the doctor's personal line. If you give us less than four hours notice there will be a $15 charge. Without any notification the charge will be $42. Insurance cannot be billed for these charges. |
